Hospital "never events" are things that are without doubt never supposed to happen. These are things such as surgery on teh wrong body part, leaving surgical tools insied a pateint, giving the wrong medication or giving the right medication but in the wrong dose or route of administration. They also include falls, bedsores, and violence against a patient. Yet, they still happen with alarming frequency and many times the patient is never told about it. According to the abstract attached here, surgical never events alone account for over 4,000 of these events every year. And, over 12% of surgeons involved with a never event will be involved with another one in the future. This does not include medication errors and teh other categories of never events.
