Hospital "never events" are things that are without doubt never supposed to happen. These are things such as surgery on teh wrong body part, leaving surgical tools insied a pateint, giving the wrong medication or giving the right medication but in the wrong dose or route of administration. They also include falls, bedsores, and violence against a patient. Yet, they still happen with alarming frequency and many times the patient is never told about it. And, over 12% of surgeons involved with a never event will be involved with another one in the future. This does not include medication errors and teh other categories of never events.
