The feds targeted some of the worst medicare frauds in a nationwide sweep in recent years. In 2012 two Dallas men were indicted in over $1M in fraudulent billings. You can read the indictment here.  These two were recently convicted of those charges and now face lengthy prison sentences. It seems these two set up multiple companies that billed medicare for monitoring patients using a third fuy who actually had a medical license.  Problem was no one ever actually monitored any patient's healthcare and fake billings were sent in to the feds even while the doc was on a cruise. These two might have gotten a leading role in the next "Dumber and Dumber(er)" flick. This is just a glimpse of some of the real drivers of high medical costs in the US.
